WE HAVE BEEN COUNTING DOWN THE DAYS AND YTH SUMMER CAMP IS FINALLY HERE! EVERY SUMMER GOD EXCEEDS OUR EXPECTATIONS AND WE LEAVE DIFFERENT THAN WE CAME. NEEDLESS TO SAY, WE COULDN’T BE MORE EXPECTANT FOR THIS YEAR’S CAMP! TRUST US WHEN WE SAY YOU ARE GOING TO WANT YOUR CHILD TO BE APART! SO GO AHEAD AND MARK YOUR CALENDAR FOR JUNE 3RD-7TH! AND WE ARE HEADED BACK TO BETHANY, LA!

WE WANT TO THANK YOU FOR ALLOWING YOUR CHILD TO JOIN US THIS SUMMER. THANK YOU FOR TRUSTING US TO INVEST AND IMPART INTO YOUR CHILD. WE ARE ALREADY COVERING THIS CAMP IN PRAYER AND INTERCESSION. WE ABSOLUTELY CANNOT WAIT TO HEAR THE TESTIMONIES OF WHAT GOD IS GOING TO DO.

My student is in sufficiently good physical condition and free from any communicable disease to engage in the trip and activities pertaining to summer camp 2025 at Camp Bethany in Bethany, Louisiana, and I give my complete, unqualified permission for him/her to do so without a health exam certified by a doctor.

I assume full responsibility for my student’s conduct, activity and well-being and hereby release and forever discharge One Life Church and Living Word Church from any and all liability arising out of my student’s participation in this trip and all activities commencing June 3rd - 7th.

I recognize that One Life Church & Living Word Church assume no responsibility for providing any type of medical coverage or insurance for participants.

In the case of an accident or emergency situation requiring medical treatment, One Life Church & Living Word Church have my permission to administer medical aid to my student as deemed necessary by the One Life Church and Living Word Church staff. I also understand that my student will not and cannot attend this function and participate without completing this form.

I hereby release One Life Church, Living Word Church, and/or all staff and volunteer leaders from fault or injury to my student which may occur as a result of efforts to secure any treatment.

I indemnify and forever hold harmless One Life Church, Living Word Church and all related entities, employees, and agents, thereof from all causes of action, claims and damages, and demands whatsoever, my student ever had, now has, or may have arising out of his/her participation in this trip. I also understand that if my student engages in conduct that would endanger his/her well-being or the well-being of any other person/persons while participating, it will require immediate dismissal from the event.

Personal Conduct & Courtesy

Failure by campers to cooperate with camp regulations will necessitate the camper returning home at parent/guardian's expense and without refund of fees. Campers sent home from camp will require special permission through their pastor/youth minister to return the following year.

1. The use of alcohol, drugs, weapons, fireworks, and offensive language is strictly prohibited.

2. In matters of dress, MODESTY must prevail. Halter tops, short shorts, etc… will not be allowed. Shirts are to be worn at all times.

3. Girls must wear a shirt over swimsuits. Boy’s swimsuits must be proper length and tightness, no speedos or basketball shorts. They must be swimming shorts with an inner lining.

4. You must have ADULT SUPERVISION at all times.

5. DO NOT deface camp property. Intentional damage or damage due to unnecessary carelessness will result in the camper being charged for damages. This includes but is not limited to moving beds, furniture, writing or putting stickers on walls/beds.

6. Armbands must be worn at all times.

7. If you break something, report it immediately.

8. Shaving cream (other than for shaving) and silly string are not allowed in any of the buildings.

9. Campers must stay in cabins after “Lights Out”.
a. Unauthorized absence from assigned building can result in disciplinary release from camp.

Student Driver Permission And Waiver Form

***THIS WAIVER IS MANDATORY IF YOUR STUDENT PLANS TO DRIVE THEMSELVES TO CAMP***
I, the undersigned parent(s)/guardian(s), permit my child to drive to and from Camp Bethany for J17 Student Camp. In granting my permission, I agree to the following:

All students are classified as “minors” for J17 Student Camp purposes even if they are over 18 years of age. I am responsible for the Student as parent/guardian.

The student has a current/valid LA state driver’s license; the vehicle’s registration, insurance and inspection are up to date, and the Student is insured on the vehicle.

The parent(s)/guardian(s) and/or the Student and/or the driver assume the risk of harm, injury of death to the Student or others, and that by voluntarily allowing the Student to operate his/her own vehicle, the parent(s)/guardian(s) and the Student will hold Living Word Church/J17 and its officers and employees free from all liability.

I understand that the Student agrees to drive safely and in accordance with all laws and regulations, on and off the camp facilities.

I understand that the Student must arrive on time, must not leave early unless given consent from a parent/guardian.

I agree that the Student is prohibited from driving other students to and from Camp without the express, written consent and release of liability from all parents/guardians involved, including myself.

I understand that the Student’s car is effectively “off limits” (no loitering in/around car) during the camp.

I agree that the Student has read and agrees to these requirements as well as the rules in the student agreement.

STUDENT AGREEMENT: I, the undersigned Student, have read and agree to the provisions above. I understand that driving to and from Camp is a privilege and carries serious responsibilities. I understand that my car is effectively “off limits” during camp (no loitering in and around). I agree, under the responsibility of my parent(s)/guardian(s), that driving is a serious responsibility, that I will obey laws and regulations on and off campus and that I will in no way abuse the privilege to drive to and from camp.
